Your process begins with an "in-home" interview by a professionally trained Case Coordinator or RN Supervisor. The Coordinator will do a complete assessment profile of your loved one, including, but not limited to, the following questions:
All the information that is gathered from the "in-home" assessment is noted by your Case Coordinator and will become part of your Personal Home Care Plan.
With your Personal Home Care Plan in hand, your Case Coordinator will match your needs with one of our trained/experienced Caring Choice Caregivers.
Whether you need constant care, have suffered a minor illness, are recuperating from surgery, or a heart attack, or another condition that requires home care assistance; Caring Choice is there for you.
We are available 24 hours a day - 7 days a week, Holidays and weekends.
A thorough home safety assessment will also be conducted as well as a nutritional and dietary evaluation. This program is at no cost to you and is part of the complimentary service provided by your Caring Choice Case Coordinator.
Our total service concept provides the best home care possible. When you make the Caring Choice, you are assured that meals are prepared the way you like them. Special dietary needs? No problem! Your food is prepared with the knowledge that your health is our number one priority.
Your home will be clean and orderly, giving you the type of comfort you deserve and expect from your care provider.
We will help remind you of your medication schedules. Gentle reminders help to speed your recovery.
Do you have hobbies or special interests? Your Caring Choice team will make every effort to help you continue enjoying those things in life, which inspire you!
Need help for those special days in the year? We can help make a meal, set the table, color those Easter Eggs or help with a Shabbat Table setting. Need help in decorating that Christmas Tree? or wrapping those presents?
